All official employment records and documents pertaining to employment are placed in each employee's document file maintained by the Operations Department.

The standard records required and responsibility for the initiation of the record and/or forwarding of the record to the Operations Department are as follows:

  1. Hiring Department

    Required records and responsibility for initiating and forwarding to the Operations Department by All Supervisors Completing Hiring
    Application Forms Recommendation for Promotion
    Employee Credential Summary Recommendation for Pay Rate Adjustment
    Letters of Acceptance Probationary Report Forms
    Pre-Testing Forms Written Disciplinary Action
    Suspension and Layoff Notices Letters of Resgination
    Leave of Absence Forms Written Reprimands

  2. Individual Employee

    Required records and responsibility for initiating and forwarding to the Operations Department by Individual Employee
    Employee Personal Data forms (as appropriate to report/change personal information)
    Training certificates, as appropriate
    Written commendations, as appropriate
    Diplomas, as appropriate

  3. Human Resources

    Required records and responsibility for initiating and forwarding to document file by The Operations Department
    Employment and Transfer applications and related documents
    Retirement and tax-deferred savings enrollment forms and related documents
    Insurance enrollment and change forms Long-Term / Short Term / Life / Disability Claims
    Employee Handbook Receipts Orientation Notice and Checklist
    Benefits Statements Dependent Eligibility Forms
    Leave of Absence forms and documents Written Reprimands
    Wage assignments, garnishments, and tax levies List / Sign off on company issued items.
