Health Insurance

Blue Cross / Highmark

After an employee’s 30-day probationary period, full time employees will be eligible to enroll in our employee health insurance program with Blue Cross / Highmark. Northeast Data will contribute a healthcare stipend for full time, enrolled employees. This plan is a benefit to the employee, however, does not need to be utilized. The employee will not be reimbursed for any portion of the stipend not utilized.

If the amount of the coverage is greater than the amount of the company healthcare stipend, said employee will have to pay the sremaining monthly cost. This amount will be deducted from the employee’s paycheck bi-weekly.

If you should decline the health insurance you must do so by signing the form where indicated and complete the Enrollment Form choosing "Waive" options where applicable

Note: Due to Obamacare regulations, you will be expected to provide proof of insurance via health card with your name or a bill in cases of individuals enrolled into the armed forces showing they are paying for insurance coverage on yourself.

Employees on short term disability will be covered for individual health care premiums for up to 12-weeks. After the 12-weeks the plan will be converted to a Cobra Plan. The employee will be offered re-enrollment after they have returned to full time employment.

Ineligible Employees

Employees not eligible for coverage include:

  • Part time employees who work less than 20 hours per week.
  • Temporary Employees
  • Seasonal Employees
  • Independent Contractors, including 1099 contractors
  • Unpaid Workers
  • Members of household staffs or personal employees
  • Employees not having satisfied the group imposed waiting period
  • Early retirees
  • Employees on long-term disability
  • Employees who have not worked for six (6) months due to illness or injury or for three (3) months due to leave of absence or
    temporary layoff.
  • Employees not scheduled to return to work.

Termination

If for any reason you separate/terminate from Northeast Data’s employment your insurance will only cover you to the end of the month that you separated employment. Northeast Data, Inc. will send you notification by mail of any cobra options that you may qualify for. VBA - Vision Program / Delta Dental - Dental Program

After an employee’s 30-day probationary period, employees will be eligible to enroll in our employee Vision Program / Dental Program. These costs are separate from the health insurance premium amount. Any employee who has extra benefits remaining from their health insurance benefit, may use up their additional benefit towards the Vision / Dental program. However, any amount exceeding the healthcare stipend will be deducted from the employee’s bi-weekly paycheck.
